IMPORTANT TERMS AND CONDITIONS TO KNOW
Covered events: The Natrelle® ConfidencePlus™ Warranty Program applies only to the following covered events (each a “Covered Event”) if they require surgical intervention:
- Rupture of Natrelle® silicone-filled breast implants due to crease-fold failure
- Loss of shell integrity from unknown cause
- Removal of implants for capsular contracture Baker Grade III/IV (product replacement only within ten years of implantation surgery)
- BIA-ALCL diagnosis
- Deflation of Natrelle® saline-filled breast implants due to crease-fold failure
- Loss of shell integrity from unknown cause
- Loss of valve integrity
- BIA-ALCL diagnosis
The Natrelle® ConfidencePlus™ Warranty Program may also apply to other event-related losses of shell integrity not specifically excluded, subject to review and approval by Allergan.
does not apply to:
- Dissatisfaction with implant size or aesthetic appearance
- Removal of implants for size alteration
- Removal of implants due to wrinkling or rippling
- Replacement of implants due to double capsule
- Loss of shell integrity caused by operative procedures
- Loss of shell integrity resulting from open capsulotomy or closed compression capsulotomy procedures
- Other manufacturers’ products
- Financial reimbursement for a non-Natrelle® replacement product in lieu of a Natrelle® replacement product
The Natrelle® ConfidencePlus™ Warranty Program may also apply to other event-related losses of shell integrity not specifically excluded, subject to review and approval by Allergan.
SPECIAL CONSIDERATIONS
Each Natrelle® breast implant is covered individually. In the case of any covered event above, you may choose to replace both implants or only the damaged implant. Coverage of the opposite (contralateral) implant will continue for the remaining life of the warranty.
TO SUBMIT A CLAIM
Your surgeon must contact the Allergan Canada Product Support Team prior to replacement surgery. The surgeon is required to return the explanted, sterilized Natrelle® breast implant(s) to assist with evaluation and maintaining the quality and safety of Natrelle® breast implants. Financial assistance will be issued upon receipt and verification of all required documentation.
